I am a 20+ years Registered Nurse in Texas.   I have spent my entire nursing career as a VA Nurse. At the VA I have worked in various clinical areas such as at the bed side, management, education, and informatics.  I am currently a Risk Manager at the VA.  Along with pursuit of the nursing doctoral degree, I have professional interest in the clinical education of nursing students. Informatics has impacted nursing practice by improving the documentation of core patient measurements, real time communication of physician orders, improved patient identification, and availability of patients’ medical records.  For example, when I first began my nursing career the nursing day centered around the availability of the patient’s paper chart.  Chart audits, carrying out physician orders, and obtaining patient labels to apply to papers forms, all centered around being able to find the chart which was “never” returned to the chart rack.  As the organization transitioned to paperless systems, data was gathered electronically, physician orders were made immediately available, and patients were able to be identified through wristband barcode matched with the electronic medical record.
Established informatics competencies have improved the quality of care, safety, and excellence in nursing practice by supporting the decision making of nurses to achieve desired outcomes (American Nurses Association, 2008).  In fact, the American Nurses Association (ANA) identified the data, information, knowledge to wisdom continuum exists with informatics competencies that directly impacts the nurse thinking and actions as the nurse interacts with patients and other healthcare disciples.
During my nursing career I have worked in clinical informatics, which deals with a broader scope of clinical outcomes and not specific to nursing outcomes.  ANA’s 2008 “Nursing Informatics Scope and Standards define nursing informatics as the integration of nursing science, computer and information science, and cognitive science to manage communication and expand the data, information, knowledge and wisdom of nursing practice (Ball, et al., 2011).” My informatics strengths and preferences lay in ANA’s definition, as the term “nursing” is easily substituted with “clinical”.  It will be interesting this semester to deep dive into nursing informatics and focus on its impact on nursing outcomes.  The Technology Informatics Guiding Education Reform (TIGER) initiative could provide potential challenges and areas for development for me, in that the TIGER Nursing Informatics Competencies Model (TICC) appears to be very prescriptive for practicing nurses.  I feel confident with the first part of the competency model that deals with basic computer literacy.  The second part of the TICC model deals with information literacy.  Information literacy could be more challenging as the need for information changes and therefore the informatist must stay current with colleagues, policies and literature (Ball, et al., 2011).  The third standard in TICC is the principle of information management.  Information management is a core component of nursing regardless of the information format.  Patient confidentiality and privacy are nursing responsibilities that have not changed with the addition of electronic health records (Ball, et al., 2011).
