Introduction
China Airlines Flight CI611 was a Boeing 747-200. It was registered with the registration number B-18255. Unfortunately, it was crashed on May 25 2002 into the Taiwan Strait. It was scheduled from Chiang Kai Shek to Hong Kong [1]. The aircraft crashed in mid-air and broke into pieces. All the 206 passengers were killed in this crash along with 19 crew members.
Reasons behind the Crash
To investigate the whole incident and to know the reasons behind this occurrence, Aviation Safety Council (ASC) appointed a team. National Transportation Safety Board of U.S.A included its team members FAA as it was the licensed representative of the manufacture. Members from the Civil Aeronautical Administration were also invited for the investigation. In the investigation it was concluded that the accident was resulted due to the metal fatigue. This happened because of improper maintenance after an accident happened previously. It was found from the report that the accident aircraft already involved in the tail strike accident previously while it was landing in Hong Kong on 7 February 1980. On the same day, the aircraft was transported back to the Taiwan and repaired temporarily. China Airlines team conducted a permanent repair but the repair done by them is not as per the Structural Manual Repair (SRM). Damaged area was not eliminated, and repeatedly done pressurization and depressurization, damaged part started getting cracked. And finally it broke on 25 may 2002 in the mid of flight [2].
The report on investigation described causes for the accident:
Based on all the recordings, it was found that accident was a result of structural failure in the aft lower lobe section.
Evidence of fatigue damage was found in the report.
Safety
Report on the aircraft accident also described 7 findings related to the risks, other than the findings related to the causes of the accident. As these risk findings were not related directly with the occurrence but these findings should be considered important equally in aviation safety point of view. The risk related findings included:
On November 1993, the first Corrosion Prevention and Control Program inspection was carried out of the accident aircraft. Its second CPCP examination was due in November 1997. But because of the reduced aircraft utilization, the flight hour inspections had become postponed and thus making the CPCP examination dates to be passed.
The number of maximum flight cycles implementation was dependent on the fatigue testing before the Repair Assessment Program. But it was not taken into consideration any standards of repair, workmanship, maintenance and follow-up inspections.
There were 640 repair doublers were found on the accident aircraft from the photographs which were taken during the structural patch survey in November 2001 for the Repair Assessment Program. Traces of stains were found on the aft lower lobe.
Proper maintenance activities were not performed before the accident[2].
Quality
The entire investigation proved that the accident was a result of negligence of safety measures and the poor quality of aircraft. The damaged part was repaired with repeated pressurization and depressurization which resulted in the metal fatigue and cracks were developed in the aft lower lobe. This resulted in the mid-air broke of the lower lobe resulting in the aircraft crash. It was basically breach of utilitarian ethics. Safety breaches were the result of the accident and life of 260 passengers along with 19 crew members were putted into danger and they lost their life in the accident [2].
